2. Please request to have any lab tests or prior examination results that are pertinent to your case faxed (303-449-3775) or sent to us prior to your consultation. You can also bring them with you to your appointment.

3. A $200 deposit is required to make a new patient appointment. It is Holistica’s policy that a deposit is required to secure your appointment. Please have your credit card ready when making your appointment. Once you come in for your initial visit, the $200 will be applied to your total balance for the visit. Please note: you must give at least 48 business hours notice of cancellation; otherwise, you will forfeit the $200.

4. We require 48 hours advanced notice for cancellation of all follow up appointments, and any missed appointments will be charged 50% of the appointment fee.

WHAT CAN YOU EXPECT?

1. Your initial appointment will take about 1-1.5 hours. It will include:

  • An in-depth discussion of your health status and your specific goals
  • A review of your past and current medical history
  • A discussion of your family history, diet, medications and supplements
  • Laboratory tests ordered or drawn in office, if necessary
  • Initial treatment plan

2. Please bring all supplements currently used to the initial appointment. If you live outside of Colorado, please fax a list of supplements and dosages to (303) 449-3775.

3. WE DO NOT ACCEPT INSURANCE, and WE DO NOT ACCEPT MEDICAID OR MEDICARE. We cannot provide health insurance claim forms (HICF) to Medicaid or Medicare patients, and these patients will not receive reimbursement for services rendered in our office. However, for patients who have insurance with any of the other major insurance carries, we are happy to provide a  HICF along with all necessary procedure and diagnostic codes so you can personally submit to your insurance company for possible reimbursement. We recommend that patients call their insurance companies ahead of time and ask about their out-of-network benefits.

4. We require 24 hours notice for all cancellations. If 24 hours notice is not given, new clients will be charged for 50% of the cost of their appointment and established patients will be charged a $75 fee. We ask for a valid credit card to hold all new patient appointments.
